It costs about 50 (including getting official copies made) and you can apply online here: They send you the document really quickly. When you get it you need to sign it with your old name and your new name in front of a witness, and that's your name changed. What you're really supposed to do then is post it back to them, and they will keep it safe for you and send you two official copies, which you can use to get your passport changed, bank account, phone contract etc. Or you can just keep the original document, which will save you about 15 by not having the copies made, but if you ever lose it you will have to pay for a new one.
